Florida Income Deduction Order Form

To clear up confusion about existing federal and Florida income deduction orders (IDOs) and notices, effective December 5, 2019, the Supreme Court of Florida adopted amended Florida Family Law Form 12.996(a) for income deduction.

Use the amended form in both Title IV-D and non-Title IV-D cases. The new form is available here (pdf) or here (rtf). For income deduction for child support, the federal government, under sections 466(a)(1), (a)(8) and 466 (b)(6)(A)(ii) of the Social Security Act, requires that states use an Income Withholding Order (IWO) for the deduction of child support. In Florida child support cases, attach the federal Income Withholding for Support Form (IWO) as a notice to new Form 12.996(a) IDO. The newly amended Income Deduction Order Florida Family Law Form 12.996(a) adds options for all forms of alimony & child support that may be deducted.
